REGISTRATION

1) Please email to volleyfest09@gmail.com to request for both the registration and indemnity forms
(we will revert back within 24 hours).

2) The organizing committee reserves the right to disallow participation of event for teams without indemnity forms.

3) Please note that each individual participant is required to have completed and submitted an indemnity form.

4) All entries should be made on official registration forms or photocopies of the original form.

5) All teams are required to make payment upon the submission of registration form.

6) Please kindly ensure that all details on the forms are filled in correctly and clearly.

7) Payment can be made at anytime before the event, but confirmation of registration will only be made upon receipt of payment.

8) Please note that registration is based on a first-come-first-serve basis.

9) Participants are required to bring down the receipt of payment AND their indemnity forms (if not submitted) during the day of the event.

10) Registration closes on 10th May 2009 OR when all 20 slots have been filled up, whichever comes first.

PAYMENT

1) Upon the completion of the forms, kindly contact Kelly (91175038) or Xing Juan (92771357) to arrange for a convenient time of transaction.

2) Participants are responsible for ensuring the collection of the receipt after payment is made.
